Short description of what you are looking for.
If you desire a look similar to your current one, please attach our favorite photos of yourself where we can clearly see your hair's length, color, and texture. If you prefer something different from your usual look, please attach BOTH your current and desired looks.

Insurance Coverage

After reading the information provided on our Insurance page, please email the following to wigsbylillianlee@gmail.com:

Your full name, phone number, and address.
Who referred you?
Type of network coverage (in or out).
Medical Insurance card, and date of birth.
Dr. prescription with Dx. ICD-10 diagnosis AND therapy code (must have 2 codes).
Your favorite photos of yourself where we can clearly see your hair's length, color, and texture. If you prefer something different from your usual look, please attach BOTH your current and desired looks. If hair loss has begun, please send before and after pictures as well.

Virtual Appointments

If you require a virtual appointment and are going through insurance, please follow the guidelines in the Insurance Coverage column, and include in your email that you need a virtual appointment along with your time zone.
